WHY H@H?

The ambition of the Hospital@Home (H@H) project is to transform healthcare delivery in Europe by enabling hospital-level care to be provided in patients’ homes. 

The project aims to:

  • Address the growing complexity and burden of chronic diseases (cardiovascular, pulmonary, minor stroke/TIA, migraine, cluster headache) by shifting care from hospitals to home environments.
  • Integrate digital health technologies (wearables, AI-driven analytics, patient-reported outcomes, digital decision support) into real-world care pathways.
  • Improve clinical and patient-reported outcomes, enhance patient experience, and reduce hospital burden, thereby supporting sustainable health systems across Europe.
  • Deliver scalable digital platforms and AI tools to improve efficiency, reduce costs, and enable sustainable business models for healthcare.
  • Provide actionable blueprints for Europe’s healthcare future, boosting patient outcomes and easing system burdens through high-quality, accessible home care. 

OUR MISSION

The mission is to place people’s needs and preferences at the centre of care transformation by:

Generating new evidence on patient-centred models by integrating patient-reported outcomes and real-world data.

Overcoming real-world barriers to digital care integration.

Delivering hospital-level treatment at home, supported by intelligent, connected systems that ensure safety, quality, and continuity without removing patients from their daily lives.

Empowering patients and healthcare professionals through engagement, ethics, equity, and system-level sustainability, woven into every phase of the project’s methodology (Mapping, Testing, Implementing).

KEY OBJECTIVES

  • Empower people and professionals: Co-design and implement engagement strategies and upskilling initiatives for 5,000+ individuals.
  • Validate integrated H@H models: Deploy and validate five condition-specific models, enrolling 3,600 patients.
  • Establish secure, interoperable digital infrastructure: Support AI-enabled decision-making and real-time monitoring.
  • Create a Digital Transformation Knowledge Hub: Consolidate validated insights and support policy adoption.
  • Define systemic pathways for sustainability: Develop frameworks for evaluation, reimbursement, and policy adoption. 
  • Empowered patients: Receive the right care, in the right place, at the right time.
  • Supported professionals: Access to digital tools, training, and evidence-based pathways.
  • Digital infrastructure: Seamless integration of technology, data, and human expertise.
  • Digital Transformation Knowledge Hub: developing and implementing a knowledge hub including insights from user experiences, technology, and clinical integration. 
  • Sustainable impact: Scalable solutions for healthier, more resilient communities across Europe.
