Global Heart Hub (GHH) and the European Migraine & Headache Alliance (EMHA) are pleased to announce a call for representatives to join a Patient Advisory Board (PAB) for the Hospital@Home project (H@H). This five-year initiative, led by Erasmus MC and Salvia BioElectronics, aims to provide hospital-level care to patients at their own homes.
The H@H project, funded by the Innovative Health Initiative (IHI) and private investments, brings together 20 partners from 10 countries, including leading university hospitals, technology companies, pharmaceutical companies and patient organisations.
H@H focuses on five major areas:
Using intelligent digital technologies, AI-enabled decision support and patient-centered care models, the project is designed to improve clinical and patient reported outcomes, enhance patient experience and reduce hospital burden to enable sustainable health systems across Europe.

The PAB will establish a structured engagement model to ensure meaningful and sustained participation of patients and informal carers in the co-design, implementation and evaluation of H@H solutions.
To ensure diversity in geography, gender, age, and digital and health literacy, we are searching for 10 individuals (patients and family members/carers of patients) in any of these disease areas: cardiovascular disease (particularly heart failure, atrial fibrillation and ischemic heart disease), pulmonary disease (chronic obstructive pulmonary disease and pulmonary fibrosis ), minor stroke/ Transient ischemic attacks (TIA), migraine and cluster headache.
While the available budget limits the level of remuneration we are able to offer, we greatly value and recognise the expertise and lived experience you bring. Participation will therefore be supported with a modest honorarium as a gesture of our appreciation.
