Use Case 1 · WP3 — Cardiovascular Care

Wearables, AI and your care team,

all connected.
Heart failure, atrial fibrillation and ischaemic heart disease are among the leading reasons people end up in hospital. H@H moves the follow-up, monitoring and early warning system into the patient’s own home.

What H@H is doing

Patients wear sensors that continuously track heart rate, breathing, oxygen levels, temperature, activity, and sleep for three to six months. This data feeds into an AI model that detects early signs of deterioration or arrhythmia and triggers a real-time alert to the care team, enabling a teleconsultation, medication adjustment or home visit before a crisis develops.
