Use Case 2 · WP4 — Pulmonary Care

Every breath monitored,

right from home.
Chronic Obstructive Pulmonary Disease (COPD) and pulmonary fibrosis (PF) are two of the most burdensome respiratory conditions in Europe. H@H builds a digitally supported home-care pathway for both – from continuous passive monitoring to real-time, traffic-light alerts for care teams.

What H@H is doing

Patients wear a device that continuously records heart rate, breathing rate, oxygen saturation, temperature, movement and sleep for up to twelve months, alongside regular symptom questionnaires. An AI-powered triage system turns this stream of data into simple green/orange/red alerts, prompting a teleconsultation, medication change or in-person follow-up exactly when it’s needed.

A first for Europe

The first comprehensive, AI-enabled home-care model for pulmonary fibrosis in Europe

Passive, continuous home monitoring – collecting rich clinical data without adding any extra burden to the patient’s day – combined with an AI early-warning (’traffic light’) system.
